About FamilyWell

FamilyWell's mission is to transform women's mental health across the reproductive journey, from fertility through menopause. We embed evidence-based, insurance-covered mental health care directly into women's health practices, making mental health support available where patients already receive care.

By integrating a virtual team of care managers, coaches, therapists, and psychiatric providers into clinical workflows, FamilyWell is expanding access to mental health support, improving outcomes for women and families, and reducing the administrative burden for our provider partners.

About the Role

FamilyWell is seeking a compassionate, organized, and proactive Patient Navigator to help women access high-quality mental health care.

As a Patient Navigator, you will serve as the first point of contact for prospective patients, helping them understand our services, navigate the enrollment process, and establish care with their FamilyWell care team. You'll work closely with our clinical and operations teams to ensure patients have a welcoming, supportive, and seamless experience.

This role is ideal for someone who enjoys building relationships, solving problems, and making a meaningful difference in patients' lives.

  • Conduct introductory calls to explain FamilyWell's services, answer questions, and help patients understand their care options.
  • Build trust and rapport with patients through compassionate, professional communication.
  • Proactively reach out to patients by phone, text, and other communication channels to support enrollment and engagement.
  • Help patients navigate barriers to accessing care.

Scheduling & Care Coordination

  • Assist patients with scheduling appointments and connecting with appropriate care providers.
  • Coordinate with clinical and operations teams to ensure a smooth patient experience.
  • Follow up with patients who miss appointments or need additional support staying connected to care.
  • Help identify and resolve scheduling challenges and other administrative barriers.
  • Support continuity of care through timely communication and follow-up.

Insurance & Administrative Support

  • Help patients understand their insurance coverage, potential out-of-pocket costs, and payment expectations.
  • Assist with collecting and verifying patient information.
  • Maintain accurate and timely documentation in the electronic health record (EHR) and other patient management systems.
  • Support administrative processes that help patients access care efficiently.

Patient Experience & Quality Improvement

  • Identify opportunities to improve the patient journey and reduce barriers to care.
  • Collaborate with team members to troubleshoot patient concerns and improve workflows.
  • Contribute to a positive, patient-centered culture focused on accessibility, responsiveness, and high-quality service.

What You Bring

Required Qualifications

  • 1–3+ years of experience in patient navigation, healthcare administration, patient experience, customer service, or a related role.
  • Excellent interpersonal and communication skills, with a warm, empathetic, and professional approach.
  • Strong organizational skills and attention to detail.
  • Ability to manage multiple priorities in a fast-paced, remote environment.
  • Comfort using technology, learning new systems, and communicating with patients by phone, text, and email.
  • A proactive, solutions-oriented mindset and a commitment to delivering an excellent patient experience.
  • Ability to work both independently and collaboratively with a multidisciplinary team.

Preferred Qualifications

  • Experience working in healthcare, behavioral health, or women's health.
  • Familiarity with health insurance benefits and patient scheduling.
  • Experience using electronic health records (EHRs) or patient management platforms.
  • Bilingual in English and Spanish.

What Success Looks Like

In this role, success means

  • Patients feel welcomed, supported, and informed throughout their experience with FamilyWell.
  • Patients are contacted promptly and connected to care in a timely manner.
  • Scheduling and administrative barriers are addressed proactively.
  • Patients who need additional support receive consistent follow-up.
  • Patient information is documented accurately and efficiently.
  • You contribute to a collaborative team environment and help continuously improve the patient experience.
