- Lead, supervise, coach, and develop LEAP's social work/clinical team, including supervision of an Associate Director of Clinical Case Management, if established, to support agency-wide clinical operations and day-to-day implementation.
- Provide clinical supervision to assigned staff and partner with an external clinical consultant to ensure appropriate clinical supervision across the team, consistent with Board of Behavioral Sciences (BBS) requirements and professional standards.
- Maintain an active presence in the field, observing staff practice, reviewing cases and plans, and coaching staff to strengthen clinical judgment and service quality.
- Monitor caseloads, workload, staff capacity, and clinical practice across regions.
- Lead case consultation and support social workers in complex cases, crises, and situations requiring higher-level clinical judgment.
- Identify training and professional-development needs for staff and providers and build team capacity to deliver LEAP's clinical model consistently.
- Case Management Implementation & Service Oversight
- Oversee implementation of clinical programs, protocols, and service standards across regions.
- Ensure clinical assessments, intake, care planning, case management, home visits, crisis planning, and service coordination are delivered consistently and appropriately in collaboration with the Residential Services department.
- Ensure clinical services match each client’s assessed needs and are effectively integrated into their care plan.
- Oversee clinical and therapeutic consultant services, ensuring appropriate referral, utilization, coordination, and quality.
- Provide oversight of crisis response and agency-wide on-call program, ensuring that appropriate protocols, escalation channels, staff readiness, and follow-through processes are in place and effective.
- Supervise and guide the Associate Director in coordinating agency-wide clinical operations and initiatives, specialist services, workflow, and day-to-day implementation of clinical standards and practices.
- Collaborate with Residential Services, Operations, Quality & Compliance, home providers, and other partners to support coordinated service delivery.
- Case Management Strategy & Program Development
- Partner with the CEO to establish LEAP's case management strategy and clinical priorities aligned with client and provider needs, organizational goals, and growth.
- Lead the ongoing development and refinement of LEAP's case management service model and related clinical supports, including clear program goals, KPIs, and quality and impact measures.
- Develop and maintain case management and clinical program guides, protocols, standards, and practice expectations that translate the model into consistent service delivery.
- Use evidence-based practices, regulatory requirements, organizational data, and client, provider, and staff experiences to strengthen case management and clinical practices.
- Identify emerging clinical needs and opportunities for new or enhanced services, interventions, and partnerships.
- Quality, Outcomes & Continuous Improvement
- Establish meaningful clinical quality, service, and outcome measures aligned with LEAP's clinical model and organizational KPIs.
- Oversee and monitor clinical documentation, service delivery, caseload trends, utilization, outcomes, incidents, and other quality and effectiveness indicators.
- Use data, case reviews, compliance findings, client, provider, and staff experience and feedback to identify trends, risks, gaps, and opportunities for improvement.
- Ensure clinical practices meet applicable regulatory, licensing, Regional Center, ethical, and organizational requirements.
- Translate findings into improvements for clinical protocols, training, staffing, services, and program design.
- Partnerships, Cross-Functional Leadership & Board Support
- Partner with department leaders to address cross-functional needs affecting clinical services, clients, home providers, and staff.
- Build and maintain relationships with Regional Centers, clinical providers, community organizations, and other strategic partners.
- Represent LEAP's clinical model, priorities, and needs in appropriate external meetings and partnerships.
